Mesothelial cyst of round ligament treatment

Objective: Surgical treatment of a mesothelial cyst of the uterine round ligament (MCURL), an uncommon entity, has been rarely documented. In this article, we present our experience with excision of MCURLs Treatment is usually surgical excision. CONCLUSION: Mesothelial cysts of the liver round ligament are extremely rare but should be taken in consideration in the differential diagnosis of intra-abdominal cystic lesions. We stress the importance of exploratory laparoscopy that can allow both diagnosis and radical surgical excision Mesothelial cyst of the round ligament is a rare finding in females, with only a few cases reported so far. Mesothelial cysts of the round ligament are rare lesions, frequently masquerading as inguinal hernias, and should be included in the differential diagnosis of any inguinal mass. Clinical findings are those of a groin mass, discomfort, and bulging. Ultrasound and CT scans often demonstrate an aperistaltic cystic mass Objective Surgical treatment of a mesothelial cyst of the uterine round ligament (MCURL), an uncommon entity, has been rarely documented. In this article, we present our experience with excision. Mesenteric cysts, a rare cause of abdominal pain, can arise from the mesentery of the duodenum to the sigmoid colon. Preoperative imaging requires careful interpretation for accurate anatomical localisation. Complete excision by either laparoscopic or open technique is the treatment of choice for symptomatic mesenteric cysts

  1. 3. Discussion. Mesothelial cysts of the liver round ligament are rare and information comes mainly from case reports. We reviewed other 5 cases published in the literature , , , , , overall male/female ratio is 1:1, mean age is 37.5 years (range 22-57 years), and cyst diameter ranges from 5 to 14 cm (mean 9.4 cm). Most cases are generally asymptomatic and represent an incidental finding.
  2. A cyst of the canal of Nuck is a developmental anomaly due to incomplete closure of the round ligament and is the equivalent of a spermatic cord hydrocele in males. It presents as a skin-coloured, asymptomatic swelling located in the inguinal area and labium majorum, resembling an inguinal hernia
  3. The treatment of choice is complete surgical excision of the cyst by enucleation from surrounding leaves of mesentery [ 2, 8 - 10 ]. This is usually easily feasible either by laparotomy or laparoscopy in appropriately selected patients [ 3, 8 - 10 ]
  4. A small cyst was found at the beginning of the inguinal canal, lying on the round ligament. Laparoscopic treatment removed all cysts, both loosed and attached to the round ligament. Finally the peritoneus of the cord and cyst attachment area was diathermocoagulated. Histological analysis diagnosed benign cystic mesothelioma

Gonadotropin stimulation during in vitro fertilization can induce cyst development in certain parts of the female reproductive system. A case report documented the development of a mesothelial cyst on the uterine round ligament of a woman after IVF stimulation Primary cysts of mediastinum include bronchogenic cysts, enteric or duplication cysts, mesothelial cysts, thymic cysts, and thoracic duct cysts [1]. Mesothelial cysts are fluid filled sacs lined by Mesothelial cells [2]. They are usually unilocular and can be found in various sites like liver [3], round ligament [4]

Paraovarian cysts occasionally can be complicated by rupture, torsion, or hemorrhage. Large or symptomatic cysts often undergo surgical resection. Smaller asymptomatic ones are treated conservatively. Given a small chance of representing neoplasm, paraovarian cystic lesions may be recommended for follow-up imaging vessels in the cyst capsule. The cyst was com-pletely separated from the round ligament, excised completely and sent for hystopatho-logical examination (Fig. 2). The patient was discharged four days after surgery. DISCUSSION. In previous reports, the histological definition of mesothelial cyst, mesothelioma and mesothelial hyperplasia has often been unclear [].To our knowledge, there has been only one similar case of mesothelial cyst in which a unilocular cyst protruding from a femoral ring was reported in a 82-year-old female [].However, the disposition of mesothelial cells was different from the. Diaphragmatic mesothelial cyst. Diaphragmatic mesothelial cysts (DMC) are derived from coelomic remnants and are lined with mesothelial cells . Mesothelial cysts may be found in several places including the diaphragm. Due to its close proximity of DMCs to liver, lung, and pleura, it may be difficult to determine the diaphragm as the source organ Benign mesothelial ovarian cysts consist of Serous cystadonema and Mucinous cystadonema. While the former develops papillary growth which may become so prolific that the cyst will appear absolutely solid and is commonly found in women aged between 30 and 40 years (about 30% malignant), the latter consist of extra large ovarian cyst or tumor (one reportedly weighed 328 lb!) The identification of cystic lesions within the retroperitoneal space is a rare event that poses clinicians the challenge of a difficult diagnosis and disease management. Retroperitoneal cystic lesions account for a group of lesions that range from common benign lesions (e.g., lymphoceles developing as a surgical complication) to rare aggressive malignant neoplasms The patient underwent open surgery: the cyst was separated from the round ligament and excised in mass and the defect was repaired with a prolene mesh. The histopathological exam confirmed the hydrocoele (calretinin positive, mesothelial cells on the inner surface)

The clinical differential diagnosis includes round ligament cysts, varicosities of the round ligament, inguinal herniation of the ovary, epidermoid cyst, and abscesses. Definitive diagnosis can be made with ultrasonography or MRI [90, 92, 94] Indeed, leiomyosarcomas of the round ligament of the uterus are extremely rare and there is only 1 case report published of leiomyosarcoma arising in the round ligament of the uterus [6]. Recurrent tumors of the round ligament are also rare. We found 1 case report of recurrent endometrial cancer stage Ia, ori-ginating in the round ligament [10] A mesothelial cyst of the round ligament is a rare cause of an inguinal mass. Definitive treatment includes open/laparoscopic excision of the cyst with high ligation of the neck up to the peritoneal pouch, along with repair of the inguinal hernia, if present. We present a rare case of a 25-year-old woman who presented with bilateral inguinolabial swelling, clinically diagnosed as bilateral irreducible inguinal hernia The histologic composition of a cyst of the canal of Nuck reflects its peritoneal origin: a variously thick fibrous outer wall containing blood vessels and occasionally smooth muscle fibers, lined on the inside with a single layer of mesothelial cells. The diagnosis of a female hydrocele is seldom made on the basis of clinical findings alone It was excised along with the round ligament. Inguinal canal repair with closure of the deep ring and the canal was done. On histological examination the wall of the cyst was lined by mesothelium surrounded by loose fibrous tissue and entrapped bundles of muscle fibers and a few blood vessels
